Credentialled Diabetes Educator’s help people with diabetes to understand the risks and benefits of lifestyle choices and treatment options. They can
work with other members of the healthcare team to achieve clinical targets, good heath and well-being.

DO I NEED A REFERRAL?
No referral is required.
If your GP has referred you through the EPC program you will need to bring the paperwork your doctor gave you.

Blood sugar meter – if you have one
Reading glasses
A friend or relative if you wish
HOW MUCH WILL IT COST?
Consultation cost and length varies according to your requirements.
People on EPC will incur a small out of pocket expense.
Pensioners and DVA will be bulked billed.
People with private health insurance extras should check with their provider about rebates.
