WHO AM I?
I am Elizabeth Cornish. I have been a registered nurse for over
20 years, working in many different health areas. I have
qualifications in Diabetes Education and am Credentialled by the
Australian Diabetes Educators’ Association. I also work in
Diabetes Education at a major tertiary hospital and in community
settings.

WHAT DO DIABETES EDUCATOR’S DO?
Credentialled Diabetes Educator’s help people with diabetes to
understand the risks and benefits of lifestyle choices and
treatment options. They can work with other members of the
healthcare team to achieve clinical targets, good heath and
well-being.
At SIA Medical Centre you can also see a podiatrist and a
dietician.

DO I NEED A REFERRAL?
No referral is required.
If your GP has referred you through the EPC program you will
need to bring the paperwork your doctor gave you.

Reading glasses
A friend or relative if you wish
HOW MUCH WILL IT COST?
Consultation cost and length varies according to your
requirements.
People on EPC will incur a small out of pocket expense.
Pensioners and DVA will be bulked billed.
People with private health insurance extras should check with
their provider about rebates.
